Things you will need for your first appointment:
- Prescription for physical therapy from your doctor. If you don’t have a prescription, we have doctors at our facility that can see you and prescribe physical therapy if it is medically necessary. Please let us know when you call if this is the case.
- Insurance card. We do participate in most insurance networks and will bill your therapy to your insurance. We also have very affordable cash rates if you choose not to bill your insurance company.
- Photo identification please.
